Welcome to the Pre-Enrollment Electronic Document for CHAMPS Charter High School. This is the first step in your enrollment process.
FIRST STEP: Please use this electronic document to provide information about your household and your student(s) when you are new to our school. Completion of these pages will confirm your intent to accept your placement at CHAMPS. YOU MUST FILL OUT ALL SECTIONS OF THE ELECTRONIC DOCUMENT COMPLETELY. Print and sign.
SECOND STEP: The following documents are mandatory for enrollment --
Printed, signed copy of the above document
Birth Certificate, Passport, or Official Birth Record (if submitting passport, provide parent/guardian photo ID)
Immunization Records (Must include TDAP vaccine administered after age 7)
Updated Report card and/or Transcript
Special Education: IEP or 504 (if applicable)
Custodial Documentation (if applicable)
THIRD STEP: Email your printed and signed pre-enrollment form and any required documents that were not uploaded to admissions@champscharter.org.
Any missing documentation will result in the delay of processing and of scheduling your student.
